Request laser access

This form will be used to determine who should have access to a laser at MQ. Note: you will not be able to fill in this form if you have not completed a laser eye test.

To access this form,

  1. Log into the OneHelp system
  2. Go to,
    1. Research
    2. Lasers
    3. Request laser access
  3. Your personal information should be prefilled by the system
  4. Select your academic supervisor by typing their name into the "Who is your academic supervisor" field and selecting their name from those show by the system.
  5. select the laser system you wish to use by using the "which laser system do you wish to use" field and,
    1. clicking in the magnifying glass icon and filtering the by the appropriate field
    2. typing in the model of laser you are requesting access to and selecting the correct one. Note: there may be a number of the same model of laser so make sure you have the right one.
    3. typing in the asset number for the laser you would like access
  6. Provide a description of the project you will be performing using this laser system. if there is insufficient detail in this section the LSO may reject the request.
  7. review the pre filled information to ensure it is correct. Contact the LSO if there is a problem.
  8. Add any additional ancillary hazards that may be associated with your project in the "Is there any additional ancillary risk associated with this laser system?" field
  9. Work through the project specific engineering controls section assigning a "Yes", "No" or 'N/A' to all fields
    1. If an answer indicates that there is a hazard, additional information will be required
      1. a risk rank should be assigned to each hazard using the hazard ranking table directly above the engineering controls section
      2. a description of the way in which you will mitigate the risk must be provided. If there is insufficient detail in this section the request may be rejected.
  10. Work through the project specific administrative controls section assigning a "Yes", "No" or 'N/A' to all fields
    1. If an answer indicates that there is a hazard, additional information will be required
      1. a risk rank should be assigned to each hazard using the hazard ranking table directly above the engineering controls section
      2. a description of the way in which you will mitigate the risk must be provided. If there is insufficient detail in this section the request may be rejected.
  11. Add any additional details that may be relevant to your project to the "Further comments regarding Administrative and Personal Controls" field.
  12. Add a request subject and description
  13. Submit the form

Once submitted the form will go to your academic supervisor and lab manager for approval. Once they have approved the application will go to the LSO for approval. If approval is granted at each level your profile will be updated to reflect that you are approved to work with this new laser.
