Laser system move/decommission

If you have to move or decommission a laser system it is important that you register this will the LSO. Laser assets will be sighted during lab inspections each year so it is important that the laser asset register is up to date at all times. To access this form,

  1. Log into the OneHelp system
  2. Go to,
    1. Research
    2. Lasers
    3. Laser system move/decomission
  3. Your personal information should be prefilled by the system
  4. select the laser system you wish to use by using the "which laser system do you wish to use" field and,
    1. clicking in the magnifying glass icon and filtering the by the appropriate field
    2. typing in the model of laser you are requesting access to and selecting the correct one. Note: there may be a number of the same model of laser so make sure you have the right one.
    3. typing in the asset number for the laser you would like access
  5. Select if you are moving or decommissioning the laser system
    1. for a move request
      1. Add the new building and room where the laser will now be housed.
      2. Work through the engineering controls section for the new location assigning a "Yes", "No" or 'N/A' to all fields
        1. If an answer indicates that there is a hazard, additional information will be required
          1. a risk rank should be assigned to each hazard using the hazard ranking table directly above the engineering controls section
          2. a description of the way in which you will mitigate the risk must be provided. If there is insufficient detail in this section the request may be rejected.
      3. Select if the PPE will be moved
        1. for No confirm that you have ordered new PPE and that it will arrive before the commencement of work
  6. Add a request subject and description
  7. Submit the form

Once submitted the form will be sent to the LSO for approval.
